Support Forums Question

I didn't know the appropriate place to post this question as it doesn't technically have anything to do with the Portal itself.

In your Support forums, I notice that you have a "form" when you click "New Topic" that has questions with text boxes/radio buttons/etc. I was simply wondering how you guys did that. Maybe someone could post a little tutorial or mod on how to accomplish something similar for my forums.
